Physical Therapy and Wellness Care

Ascend Physical Therapy & Wellness takes a holistic approach to your treatment, meaning our goal is to assist you in feeling whole, healed and as pain free as possible. We will combine traditional physical therapy with a wellness program so you can be involved on your personal road to wellness for the future.

You will have the same physical therapist throughout your treatment with our clinic to ensure consistency of therapy and the opportunity to create a comfortable relationship. You deserve to live life to the fullest and we want to help you in every way we can!

 

What to expect on your first appointment

No two patients are exactly alike and no two people heal the same, even if they have similar conditions. Your road to wellness starts with a comprehensive evaluation of your condition or injury. We will examine your medical history. Your range of motion will be tested, along with your balance and coordination, posture, muscle performance and motor function. This allows us to customize a program of therapy geared specifically to your needs.

We will discuss the proposed course of action with you, explain our strategy, and project its anticipated outcome.

You can expect your treatment to start during this first visit, so please wear or bring comfortable, loose-fitting clothing that will allow the therapist to properly assess your problem areas. Exercise wear such as sweats, t-shirts and shorts are recommended attire.

Please plan 60 to 90 minutes for your first appointment.

After your treatment, you will be assigned some home exercises.

 

What to do in advance of your first visit

We are pleased that you have chosen our clinic, and our holistic approach is geared to treating you with care and compassion as our guest on the road to wellness. There are some things you can do in advance of your first meeting with us that will allow us to move forward with your therapies as quickly as possible.

To make your first visit go smoother, please print and complete the new patient paperwork ahead of time. For your convenience, the new patient paperwork can be downloaded on our Forms web page, or by clicking on Forms in the menu bar. If it is not possible for you to fill out the forms ahead of time, we understand. In that case, however, please come to our clinic 20 minutes before your schedule appointment time to allow this work to be done in our office.

 

Please bring the following items to your first visit:

  • Physician's referral/ prescription. Don't have a physician referral/prescription? Print out our convenient Referral/Prescription Form and ask your physician to fill it out
  • A list of any questions you may have about your injury or treatment options
  • Your health insurance card. We will need to make a photocopy of this card to ensure we collect all necessary information to bill your insurance company
  • If you are visiting our clinic for a Worker’s Compensation Injury, please bring any information you have regarding your claim: claim number, adjustor’s name and contact information)
  • Co-payment, if applicable

 

For repeat visits

Your therapist will advise you how long each follow-up appointment will be (usually 45-60 minutes).

Ascend Physical Therapy & Wellness is open Monday to Saturday with early morning and evening appointments available. You may visit us on referral from your doctor, requesting an appointment via our convenient online form, or by simply calling us and requesting a consultation.

After completion of treatment

After completing physical therapy treatment, you may wish to join the many patients who continue on with a supervised wellness program designed around your personal goals. This program can also include a one-on-one session with a therapist.
